Course Description

This course orients students to the clinical environment and provides experience with basic skills necessary to the surgical technologist. Topics include, but are not limited to: scrubbing, gowning, gloving, and draping; assistance with patient care and processing of instruments and supplies. In addition, introduces the development of surgical team participation through clinical experience. Emphasis is placed on observation and/or participation in routine procedures for core and specialty surgery. Surgical specialties include: General, Cardiothoracic, Genitourinary, Neurologic, Obstetric and Gynecological Surgery, Urological Surgery, Oral and Maxillofacial Surgery, Orthopedic Surgery, Peripheral Vascular Surgery, Plastic and Reconstructive Surgery, Procurement/Transplant Surgery, etc. Case Requirement: A student must complete a minimum of 120 cases as delineated below. A. General Surgical 1. A student must complete a min. of 30 cases in General Surgery. a) 20 of these cases must be performed in the 1st Scrub role. b) The remaining 10 cases may be performed in either the 1st Scrub or 2nd Scrub role. B. Specialty Surgery 1.A student must complete a min. of 90 cases in various surgical specialties, excluding General Surgery. a) A min. of 60 cases must be performed in the 1st Scrub role and distributed amongst a min. of 4 surgical specialties. 1) A min. of 10 cases in 4 different specialties must be completed in the 1st Scrub role (40 cases total). 2) The additional 20 cases in the 1st Scrub role may be distributed amongst any 1 surgical specialty or multiple surgical specialties. b) The remaining 30 cases may be performed in any surgical specialty in either the 1st Scrub or 2nd.

Course Outcomes

First Scrub Role

  • The student surgical technologist shall perform the following duties during any given surgical procedure with proficiency. The following list is provided to identify the items that must be completed in order to document a case in the first scrub role. As students not meeting the five criterial below cannot count the case in the first scrub role and the case must be documented in the second role or observation role.

  • Establish or verify supplies and equipment needed for the surgical procedure.

  • Establish or set up the sterile field with instruments, supplies, equipment, medication(s) and solutions needed for the procedure

  • Perform counts with the circulator prior to the procedure and before the incision is closed.

  • Demonstrate how to pass instruments and supplies to the sterile surgical team members during the procedure.

  • Demonstrate how to maintain sterile technique as measured by recognized breaks in technique and demonstrate knowledge of how to correct with appropriate technique.

Second Scrub Role

  • Perform in the observation role. The observation role is defined as the student who is in the operating room performing roles that do not meet the criteria for the first or second scrub role. These observation cases are not to be included in the required case count, but must be documented by the program.

Surgical Technologist in the First Scrub Role: Preoperative

  • Include donning OR attire.

  • Include basic hand wash.

  • Include back table:

    • Open sterile pack

    • Arrange supplies and instruments

    • Assemble a Balfour abdominal retractor.

  • Include open a sterile basin on ringstand.

  • Include instrument set:

    • Open wrapped set

    • Open container system

    • Remove instrument set from container system

  • Include open sterile supplies:

    • Small wrapped package placed onto sterile field

    • Small wrapped package secured by surgical technologist

    • Peel pack

  • Include pour sterile solution.

  • Include surgical scrub.

  • Include gown and glove self.

  • Include Mayo stand set-up:

    • Drape

    • Arrange instruments and supplies

    • Construct a sponge stick

    • Load and unload scalpel blade on scalpel handle

  • Include suture and ties:

    • Transfer suture packets to the sterile back table

    • Straighten sutures

    • Cut ties in ¼, 1/3, and ½ lengths.

  • Include fill bulb syringe.

  • Include draw up medications:

    • Vial held by circulator

    • Receive medication(s) onto sterile field

    • Label medications

  • Include gown and glove another person.

  • Include drape patient:

    • Laparotomy

    • Vagina

    • Extremity – leg

Surgical Technologist in the First Scrub Role: Intraoperative

  • Include recognize, prepare, and pass instruments.

  • Include suture and ties:

    • Load and pass NH swagged needle: right- and left-handed surgeon

    • Load free needle onto NH; thread suture; pass; and reload

    • Tag and cut sutures

    • Pass ties: free hand; instrument

  • Include contaminated sterile attire:

    • Re-glove

    • Re-gown

Surgical Technologist in the First Scrub Role: Postoperative

  • Include dressings:

    • Assemble and apply abdominal dressings

    • Assemble and apply a Montgomery strap dressing

  • Include connect ostomy bag to stoma site.

  • Included drains:

    • Connect Hemovac

    • Connect Jackson-Pratt

    • Connect chest tube to chest drainage system

  • Include remove sterile gown and gloves.

  • Include disinfecting OR:

    • End of case

    • Terminal

Surgical Technologist in the First Scrub Role: Perioperative

  • Include sponge, sharp, and instrument counts.

Assistant Circulator Role

  • Include turn and tie sterile gown.

  • Include transporting patient:

    • Prepare patient stretcher

    • Transport patient from ward room to Prop, Holding, or OR

  • Include transferring patient:

    • Stretcher to OR table

    • OR table to stretcher

  • Include taking vital signs:

    • Temperature

    • Pulse

    • Respirations

    • Blood Pressure

  • Include Electro-surgery

    • Positon grounding pad

    • Connect Bovie pencil cord to ESU

  • Include Sellick’s Maneuver
